Case Manager
You need to be a qualified health and social care professional, such as an occupational therapist, social worker, nurse, physiotherapist or clinical psychologist, and have at least eight years post qualification experience.
Your experience must include supporting clients in community settings with complex neurological problems, trauma orthopaedic conditions, or major limb loss.
We are currently looking for case managers who are based in the South Midlands, London and South East regions.
The size of your case load will depend on the hours you work and the complexity of the clients' needs, but will be relatively small to ensure a high quality and responsive service.
