North Star Case Management offers a bespoke service to individuals with complex injuries and health conditions

We are a professional and welcoming group of case managers with a proactive and responsive approach to our work

The case managers actively support clients, and their families, to manage the consequences of complex long term injuries and conditions. They have extensive experience of working with individuals with acquired brain injury, cerebral palsy, spinal cord injury, orthopaedic trauma and limb loss.

Our aim is to promote clients’ recovery, maximise their potential, facilitate functional independence, and enable a quality of life that brings fulfilment and life satisfaction.

Initially a case management assessment is undertaken and a rehabilitation plan is devised. Case managers then coordinate services to meet the clients’ needs in areas such as care and support, therapies and healthcare, home adaptations, specialist equipment, education, vocation, social participation and leisure.
